We now have our Model Contest Entry Form posted online for your convenience. We recommend that you fill out the form online, print the form, and bring the form with your model to the contest. Alternately, you can print the form, fill in the details by hand, and bring it with you to the contest. Lastly, if for some reason you can't open our Adobe Acrobat online form, we've got a form you can print that should suffice.

Instructions:

1. Click on the navigation link below titled Online Model Contest Entry Form.
2. Fill out the form FOR EACH MODEL you intend to display at the show or enter in the model contest.
3. PRINT the online form FOR EACH MODEL before you press the Reset/Clear Form button.
4. After printing the form got back to the top of the form and press the Reset/Clear Form to clear the information you entered for the previous model. This clears all the data fields except that remain constant on each of your entries (name, city, etc).
5. PLEASE don't forget to print your completed forms on your personal printer before clicking the Reset/Clear Form button!
