This little model is a Ford Aerostar and it is scratchbuilt with the exception of the grille and wheels. The grille is from a Life-Like Chevrolet Citation and the rims are from an Atlas F-150. You might remember this one from the background of Michael's junkyard in Photo Galleries 162 and 164.
